The role runs across two structured days each week \u00e2\u0080\u0094 an in-clinic day at our Ipswich partner practice and a remote telehealth case-conferencing day \u00e2\u0080\u0094 with some flexibility on the days to suit you.<\/p>\n<p>This is not a standard locum. You will provide clinical leadership within integrated Chronic Care Teams, working alongside allied health professionals and specialists to translate clinical findings into coordinated, evidence-based care plans. The work is structured, purposeful and genuinely multidisciplinary \u00e2\u0080\u0094 a system that values thorough, evidence-based care over high-volume throughput.<\/p>\n<\/p>\n<p>Remuneration<\/p>\n<p>First 3 Months (Guaranteed) = $240.00 per hour<\/p>\n<p>After 3 Months = 70% of billings<\/p>\n<ul>\n<ul>\n<li>Structured as an independent contractor arrangement.<\/li>\n<li>Bulk-billing model across Medicare and DVA billing streams.<\/li>\n<li>Sign-on bonus and anniversary incentive available (details on enquiry).<\/li>\n<li>Ipswich is a DPA-eligible location \u00e2\u0080\u0094 GPs with a 19AB exemption, including international medical graduates, are welcome to apply<\/li>\n<\/ul>\n<\/ul>\n<p>Location &amp; working options<\/p>\n<p>In-clinic day (Tuesday): an 8-hour clinic day at a well-established allied-health practice and a natural multidisciplinary home for chronic-care work.<\/p>\n<p>Remote case-conferencing day (Thursday): an 8-hour telehealth case-conferencing session, delivered from your home office (a suitable space and reliable internet are all that\u00e2\u0080\u0099s required). There is some flexibility on which days these fall, arranged around your availability.<\/p>\n<\/p>\n<p>The Role<\/p>\n<p>As a General Practitioner within our Chronic Care Team model, you will provide clinical leadership across in-clinic consultations and structured telehealth case conferencing:<\/p>\n<ul>\n<li>Conduct comprehensive GP consultations for acute, chronic and complex conditions within a Chronic Care Team framework (in-clinic and\/or by telehealth).<\/li>\n<li>Deliver preventative health assessments \u00e2\u0080\u0094 cardiovascular risk, metabolic health, mental-health screening and lifestyle risk modification.<\/li>\n<li>Undertake chronic disease management (CDM) reviews and care planning, including GP Management Plans (GPMPs) and Team Care Arrangements (TCAs).<\/li>\n<li>Lead and participate in scheduled telehealth case conferences as part of multidisciplinary Chronic Care Teams, integrating clinical, pathology, imaging and allied-health input into coordinated care plans.<\/li>\n<li>Initiate, review and manage pathology and imaging investigations; provide prescribing and medication management; and refer to allied health, specialists and community services.<\/li>\n<li>Ensure clinical safety, prioritisation and escalation pathways are clearly identified and actioned.<\/li>\n<\/ul>\n<p>Working pattern: the role is built around two structured days each week \u00e2\u0080\u0094 an 8-hour in-clinic day on a Tuesday at our Ipswich practice (consultations and chronic-disease reviews) and an 8-hour remote case-conferencing session on a Thursday (approximately 22 patient reviews, conducted entirely via telehealth). There is some flexibility on which days these fall, arranged around your availability.<\/p>\n<\/p>\n<p>Veteran Health &amp; DVA Focus<\/p>\n<p>Veteran Pathways &amp; Proactive Care has a dedicated focus on veteran health and chronic disease management for both Medicare and DVA-funded patients. You will work with veterans who often present with complex, multi-system conditions including chronic pain, mental health comorbidities, musculoskeletal injuries, and the long-term effects of military service.<\/p>\n<p>Experience with DVA billing, the Coordinated Veterans\u00e2\u0080\u0099 Care (CVC) Program, or veteran-specific health assessments is highly regarded but not essential \u00e2\u0080\u0094 comprehensive onboarding and support will be provided.<\/p>\n<p>Essential<\/p>\n<ul>\n<li>Medical degree (MBBS or equivalent) and current registration with the Medical Board of Australia (AHPRA).<\/li>\n<li>Fellowship with RACGP or ACRRM (VR GP), OR eligibility for a 19AB exemption in a DPA location such as Ipswich (IMGs welcome).<\/li>\n<li>Experience in chronic disease management, preventative health and complex care coordination.<\/li>\n<li>Competence in prescribing, referrals and investigation management.<\/li>\n<li>Strong communication and collaboration skills within multidisciplinary teams.<\/li>\n<li>For the telehealth pathway: a suitable home-office environment and reliable internet connection.<\/li>\n<\/ul>\n<p>Desirable<\/p>\n<ul>\n<li>Experience with telehealth consultations and digital-health platforms.<\/li>\n<li>Experience within team-based or multidisciplinary care models.<\/li>\n<li>Interest or experience in veteran health, DVA processes or the CVC Program.<\/li>\n<li>Experience with GP Management Plans (GPMPs), Team Care Arrangements (TCAs) and chronic disease care planning.<\/li>\n<\/ul>\n<ul>\n<li>Guaranteed $240\/hr for the first 3 months, moving to 70% of billings thereafter.<\/li>\n<li>Sign-on and anniversary bonuses to support your transition (details discussed at interview).<\/li>\n<li>A balanced two-day rhythm \u00e2\u0080\u0094 one in-clinic day at Ipswich and one remote case-conferencing day \u00e2\u0080\u0094 with some flexibility on the days, arranged around you.<\/li>\n<li>Meaningful, purpose-driven work improving outcomes for veterans and patients with complex chronic conditions.<\/li>\n<li>Multidisciplinary collaboration within established Chronic Care Teams, with full administrative, nursing and allied-health support.<\/li>\n<li>A structured clinical model that values thorough, evidence-based care over high-volume throughput.<\/li>\n<\/ul>\n<p>Apply now or contact our Head of Talent, Stuart Smith (stuart.s@pro-active.com.au) for a confidential discussion.<\/p><\/p>\n","protected":false},"excerpt":{"rendered":"<p>Are you a GP looking for meaningful, flexible clinical work \u00e2\u0080\u0094 one day in clinic, one day remote, without the full-time grind?
